Population-Based Management

This approach also recognizes the influence of social determinants of health (SDOH) and aims to create tailored solutions that address the specific needs of different populations.
According to the National Committee for Quality Assurance (NCQA), effective population health management programs should include critical components such as data integration, population assessment, and targeted interventions 2 . These elements are crucial for identifying health trends, stratifying populations by risk, and implementing appropriate health strategies.
